ABSTRACT

In vitro fertilization (IVF) is the most successful and efficient approach to infertility treatment. Choosing the appropriate embryos for transfer significantly decreases the pregnancy failure rate and improves IVF results. Invasive procedure to acquire embryo DNA has a significant limitation. The differences of metabolites, nucleic acids, proteins and other components in embryo medium can help to screen embryos with potential development to improve implantation rate and pregnancy rate, reduce abortion rate and avoid the transfer of aneuploid embryos and the transmission of mutated genes to offspring. The metabolomics screening of high developmental potential embryos with embryo culture medium can assess the developmental potential of embryos comprehensively, objectively and noninvasiveness, which is crucial for improving the pregnancy rate of single embryo transfer.

ABSTRACT

Objective:To explore the diagnostic efficacy difference and clinical diagnostic value of chronic lymphocytic leukemia flow (CLLflow) score and Moreau score (MS) in the diagnosis of chronic lymphocytic leukemia (CLL).Methods:According to the latest international and national diagnosis criteria for CLL, 133 patients with B-cell chronic lymphoproliferative diseases and uncertain immunophenotypes (B-CLPD), diagnosed by Zhengzhou Jinyu Comprehensive Haematological Pathology Diagnosis Centre from March 2020 to May 2021, were included in this study. Above patients were divided into the CLL group ( n=83) and non-CLL group ( n=50). The expression of clusters of differentiation (CD)5, CD10, CD20, CD19, κ light chain, λ light chain, FMC7, CD23, CD22, surface immunoglobulin M, CD200 and CD79 were detected by flow cytometry, and CLLflow score and MS score were calculated respectively according to the scoring rules. A fourfold table was used to compare the diagnostic efficacy of the two scoring systems, and the Kappa test and McNemar test were used to compare the consistency and superiority of the systems. Results:The rate of negative and positive CLLflow score were 4.8% (4/83) and 95.2% (79/83) in the CLL group and were 80.0% (40/50) and 20.0% (10/50) in the non-CLL group, and respectively (both P<0.001). The MS score (≤2, =3 and≥4) was 1.2% (1/83), 10.8% (9/83) and 88.0% (73/83) in the CLL group and was 86.0% (43/50), 14.0% (7/50) and 0 in the non-CLL group, there were significant statistical difference between the two groups ( P<0.001). The sensitivity, specificity, positive predictive value and negative predictive value of the CLLflow score were 95.2% (79/83), 80.0% (40/50), 88.8% (79/89) and 90.9% (40/44), respectively and those of MS score were 98.8% (82/83), 86.0% (43/50), 92.1% (82/89) and 97.7% (43/44) respectively. The overall coincidence rate, positive and negative coincidence rate between the CLLflow score and MS score were 91.0% (121/133), 93.3% (83/89) and 86.4% (38/44) respectively. Besides, the McNeamr dominance test presented no significant difference ( P>0.05) and high consistency (Kappa=0.796) between the two scoring systems. With MS≤2 and MS≥4, the sensitivity and the specificity of the MS score were 100% (73/73) and 97.7% (43/44) respectively, and for the CLLflow score, the sensitivity and the specificity were 97.3% (71/73) and 86.4% (38/44) in this MS range. With MS = 3, the sensitivity and specificity of the MS score were 100% (9/9) and 0 (0/7), and CLLflow was 88.9% (8/9) and 57.1% (4/7). Conclusions:The diagnostic efficacy is similar and presents high consistency between the CLLflow score and MS score in CLL diagnosis. For CLL patients with MS = 3, the specificity of MS is relatively low, combined assessment with CLLflow score could improve the diagnosis efficacy for CLL in these patients.

ABSTRACT

In the past few years, with the breakthrough development of DNA sequencing technology, preimplantation genetic testing (PGT) has been widely used as an important part of assisted reproductive technology (ART). The progress of next generation sequencing (NGS) technology and the improvement of resolution, as well as the comprehensive application of molecular diagnostic technology makes it possible to perform the extensive and comprehensive chromosome screening and the clinical valuable detection of small gene fragment missing and repetition simultaneously, which is of great significance in terms of improving the pregnancy rate and reducing the abortion rate, multiplets rate and malformation rate. The common PGT molecular diagnostic techniques involves fluorescence in situ hybridization (FISH), single nucleotide polymorphism array(SNP-array), quantitative polymerase chain reaction(qPCR) and NGS. And each of them has its own highlights in clinical application and there are many uncertainties that are difficult to control. Moreover, ethical concerns brought about by technological progress also need to be addressed.

ABSTRACT

Different fragments of SARS-CoV-2 nucleocapsid (N) protein were expressed and purified, and a fluorescence immunochromatography method for detection of SARS-CoV-2 total antibody was established. The effect of different protein fragments on the performance of the method was evaluated. The N protein sequence was analyzed by bioinformatics technology, expressed in prokaryotic cell and purified by metal ion affinity chromatography column. Different N protein fragments were prepared for comparison. EDC reaction was used to label fluorescence microsphere on the synthesized antigen to construct sandwich fluorescence chromatography antibody detection assay, and the performance was systemically evaluated. Among the 4 prepared N protein fragments, the full-length N protein (N419) was selected as the optimized coating antigen, N412 with 0.5 mol/L NaCl was used as the optimal combination; deleting 91-120 amino acids from the N-terminal of N412 reduced non-specific signal by 87.5%. the linear range of detection was 0.312-80 U/L, the limit of detection was 0.165 U/L, and the accuracy was more than 95%. A fluorescence immunochromatographic detection method for analysis of SARS-CoV-2 total antibody was established by pairing N protein fragments. The detection result achieved 98% concordance with the commercially available Guangzhou Wanfu test strip, which is expected to be used as a supplementary approach for detection of SARS-CoV-2. The assay could also provide experimental reference for improving the performance of COVID-19 antibody detection reagents.

ABSTRACT

Objective:To investigate the efficacy of peritoneal perfusion combined with intravenous and oral chemotherapy in the treatment of gastric cancer with peritoneal metastasis and its correlation with prognosis. Methods:The clinical data of 142 patients with peritoneal metastasis of gastric cancer admitted to the Affiliated Hospital of Xuzhou Medical University from March 2012 to June 2019 were retrospectively analyzed.According to different treatment schemes, the patients were divided into observation group (47 cases treated with tegafur capsule orally+ docetaxel+ cisplatin combined with recombinant human tumor necrosis factor intraperitoneal perfusion) and control group (oral administration of tegafur capsule+ oxaliplatin). The clinical efficacy and prognostic factors were compared between the two groups. Results:The short-term efficacy after two courses of chemotherapy: the effective rate was 44.7% (21/47) in the observation group and 8.4% (8/95) in the control group, the difference was statistically significant (χ 2=25.438, P<0.001); the disease control rate was 97.9% (46/47) in the observation group and 60.0% (57/95) in the control group, the difference was statistically significant (χ 2=22.639, P<0.001). Long term efficacy: the progression free survival time of the observation group was higher than that of the control group (6.50 months vs.3.83 months, χ 2=16.399, P<0.001); the total survival time of the observation group was higher than that of the control group (15.60 months vs.10.83 months, χ 2=9.219, P=0.002). Univariate analysis showed that ascites volume, chemotherapy, palliative surgery, liver metastasis, Lauren classification and Her-2 expression were related to prognosis (all P<0.05); there was no significant difference in cumulative survival rate of different gender, age and ECOG score (all P>0.05). Multivariate analysis showed that liver metastasis ( HR=1.996, 95% CI=1.194-3.338, P=0.008 ), Lauren type ( HR=1.979, 95% CI=1.265-3.094, P=0.003 ), Her-2 expression ( HR=3.872, 95% CI=2.202-6.808, P<0.001 ), chemotherapy regimens(whether combined with intraperitoneal chemotherapy) ( HR=1.826, 95% CI=1.046-3.188, P=0.034 ) and abdomina volume ( HR=2.785, 95% CI=1.644-4.718, P<0.001 ) were independent risk factor for the prognosis of patients. Conclusion:Intraperitoneal perfusion combined with intravenous and oral administration has a good clinical effect in the treatment of peritoneal metastasis of gastric cancer, which can effectively improve the clinical symptoms and improve the survival rate of patients. The presence or absence of liver metastasis, Lauren classification, Her-2 expression, chemotherapy method (combined with intraperitoneal perfusion or not) and ascites volume are independent risk factors for the prognosis of peritoneal metastasis of gastric cancer.

ABSTRACT

Objective To observe the expression of TLR4 in hepatocellular carcinoma cell line HepG2 after transient and stable HBV genome transfection. Methods Immunofluorescence flow cytometry was used to detect mean fluorescence intensity (MFI) of TLR4 and TLR4 positive cell percentage in hepetocellular carcinoma cell lines HepG2 and HepG2. 2.15. Various doses of HBV DNA plasmid were transfected into HepG2 cells with lipefectamine 2000. Immunofluoroscenee flow cytometry was used to detect MFI of TLR4 and TLR4 positive ceil rate of infected HepG2 ceils. Trypan blue staining was used to examine the sum of living cells. Results MFI of TLR4 and TLR4 positive cell rate of HepG2.2.15 cells were significantly higher than those in HepG2 cells (both P' <0. Ol). MFI of TLR4 and TLR4 positive cell rate of HepG2 cells transfected by various doses of HBV DNA were significantly higher than those in control group (all P' < 0. 01). MFI of TLR4 and TLR4 positive cell rate of infected HepG2 cells were positively correlated with the doses of HBV DNA (both P' <0. 01) and negatively correlated with the sum of living cells (both P' <0. 01). Conclusions Enhanced expression of TLR4 appeared in HepG2 cells with both transient and stable HBV infection, along with reduction of living cells.

ABSTRACT

Objective To study the effects of Fructus ligustri lucidi and its monomers, tyrosol and oleanotic acid, on the migration of mouse melanoblast cell line (NCCmelb4M5). Methods Cultured NCCmelb4M5 cells were treated with Fructus ligustri lucidi (0.0625, 0.125, 0.25, 0.5, 2 mg/mL), tyrosol (0.02, 0.04, 0.08, 0.16, 0.8 mg/mL) and oleanolic acid (0.0625, 0.125, 0.25, 0.5, 2.5 mg/mL), respectively, for 48 hours followed by the detection of cell proliferation with MTT assay. The working concentration of the three drugs was determined according to the results of MTT assay. Scratch and transwell assays were performed to observe the effect of Fructus ligustri lucidi and its monomers at working concentration on the migration of NCCmelb4M5 cells. Results Based on the results of MTT assay, the working concentration of Fructus ligustri lucidi, tyrosol and oleanolic acid was determined at 0.125 mg/mL, 0.08 mg/mL and 0.0625 mg/mL respectively, and at these concentrations, these drugs exhibited a cytotoxity lower than that of absolute alcohol with no obvious stimulation of cell proliferation. Scratch and transwell assay revealed a promoting effect of both Fructus ligustri lucidi and tyrosol on melanoblast migration (P<0.05), while oleanolic acid had little effect on melanoblast migration. Conclusions The extract of Fructus ligustri lucidi has a significant stimulatory effect on the migration of mouse melanoblasts, and tyrosol may be an active component of Fructus ligustri lucidi associated with confirmative effect on migration of mouse melanoblasts.

ABSTRACT

Objective To investigate the level of NF-E2 related factor 2 (Nrf 2) in vitiligo lesions. Methods Tissue samples were obtained by press suction blisters at lesional and donor sites of 12 patients with vitiligo who were managed with epidermal transplantation. Four lesional samples from the patients were subjected to primary culture and the level of Nrf 2 was detected by AEC immunohistochemistry after 48hours of culture. Western blotting was utilized to further detect the level of cytoplasmic and nuclear Nrf 2 in tissue samples from the other 8 patients with vitiligo. Results Immunohistochemistry revealed that Nrf 2 was predominantly expressed in cytoplasm, rather than nuclei, of keratinocytes in vitiligo lesions compared with the normal skin of patients. The level of nuclear Nrf 2 was significantly lower in lesions than that in normal skin (0.10 ± 0.03 vs 0.26 ± 0.03, P < 0.01) of the patients. In contrast, there was no significant dif- ference in the level of cytoplasmic Nrf2 between lesional and normal skin (0.61 ± 0.03 vs 0.60 ~ 0.02, P >0.05) of patients. Conclusion These results reveal an abnormality of nuclear translocation of Nrf 2 in vitili-go lesions.

ABSTRACT

Objective To study the effect of matrine on the expression of a proliferation-inducing ligand (APRIL) in colorectal cancer cell line (SW480 cell). Methods MTT assay was used to evaluate the inhibitory effect of matrine on SW480 cells. The protein and mRNA levels of APRIL in SW480 cells were determined by immunohistochemistry and real-time fluorescence quantitative PCR (RFQ-PCR). SW480 cells were treated with 0.5,1.0,2.0 mg/ml of matrine for 24 h, 48 h and 72 h. FU and blank were served as drug control and blank control groups, respectively. Results Matrine had obviously inhibitory effect on proliferation of SW480 cells in a time- and dose-dependant manner. The expression of APRIL was strong in SW480 cells. When treated with 50,100,200 ug/ml of FU, the APRIL mRNA levels in SW480 cells raised gradually and reached the highest levels at 72 h after treatment, which were significantly higher than those in blank control group (all P value<0.001). When treated with 0. 5,1.0, 2.0 mg/ml of matrine, the APRIL mRNA levels in SW480 cells increased at 24 h after treatment, which were significantly higher than those in blank control group (all P value<0. 001), and then decreased gradually and almost equal to level of blank control group at 72 h. Conclusion In treatment with FU, the survival cells.may have stronger ability of proliferation due to higher expression of APRIL in SW480 cells. Anti-APRIL therapy might be an important assistant treatment to counter the impact of APRIL. Matrine will not cause persistent increase of APRIL mRNA levels in SW480 cells, so it might be a helpful drug in anti-tumor theraphy.

BACKGROUND: The increase of leukocyte and blood glucose levels is a kind of stress reaction when acute cerebrovascular disease occurs, it often occurs at the early clinical stage of acute cerebrovascular disease.OBJECTIVE: To observe the levels of leukocyte and blood glucose at the early stage of acute cerebrovascular disease, and investigate their correlation with recent prognosis.DESIGN: A prognostic analysis based on diagnosis taken patients as observational subjects.SETTING: Qingtian County People's Hospital.PARTICIPANTS: Totally 235 inpatients with acute cerebrovascular disease aged (63.13±9.55) years were selected from Qingtian County People's Hospital between May 2000 and May 2002, including 90 cases of cerebral hemorrhage (52 males and 38 females) aged (63.13±9.55) years and 145 cases of cerebral infarction (73 males and 72 females) aged (71.22±8.00) years. The disease course ≤ 1 month. According to the number of leukocytes, the patients were divided into normal leukocyte group (n=167) and increased leukocyte group (n=68). According to the level of blood glucose, they were divided into normal blood glucose group (n=146) and hyperglycemia group (n=89). The neurologic deficit score (NDS) of 0-15 points was taken as mild, 16-30 as moderate and 31-45as severe.METHODS: All the patients received cranial CT examination immediately after admission, and fasting venous blood samples were drawn on the next morning to detect the levels of leukocyte and fasting blood glucose. The x2 test was applied.MAIN OUTCOME MEASURES: ① Comparison of age: It was obviously higher in the cerebral infarction group than in the cerebral hemorrhage group [(71.22±8.00), (63.13±9.55) years, t=6.90, P < 0.001]. ② Comparison of the percentage of disease types: In the increased leukocyte group, the percentage of cerebral hemorrhage was obviously higher than that of cerebral infarction (66%, 34%, x2=31.47, P < 0.005). In the hyperglycemia group, the percentage of cerebral hemorrhage was higher than that of cerebral infarction (51%, 49%, x2=9.12, P < 0.005). ③ Comparison of the percentage of severe neurological impairment: It was obviously higher in the increased leukocyte group than in the normal leukocyte group (26%, 6%,x2=21.12, P < 0.005), also markedly higher in the hyperglycemia group than in the normal blood glucose group (27%, 5%, x2=21.70, P < 0.005).④ Comparison of the percentage of death: It was obviously higher in the increased leukocyte group than in the normal leukocyte group (31%, 6%,x2=26.15, P < 0.005), also markedly higher in the hyperglycemia group than in the normal blood glucose group (26%, 5%, x2=20.02, P < 0.005). CONCLUSION: Both the increase of leukocyte and blood glucose can damage the brain tissue, and can be used to judge the disease condition of patients with acute cerebrovascular disease, estimate the prognosis and guide the treatment.
